Nursing Home Ownership Change Monitor
Recorded nursing home changes of ownership rose from 99 in 2016 to 411 in 2023, an increase of 315.2% across the seven states WhoOwnsCare covers. 2,396 changes of ownership are recorded in total between 2016 and 2026. A single portfolio transaction can appear as several facility-level records, so these are records of change, not deal counts.
Read the recent years with care. CMS records a change of ownership when the enrollment record is updated, which lags the transaction. The two most recent years in this series are therefore incomplete and will rise in later editions. The fall shown at the end of the series is a reporting artefact, not a fall in transaction activity.

What This Measures, and What It Does Not
Each row counts CMS enrollment records showing a change of ownership taking effect in that year, for Medicare-certified providers in California, Texas, Florida, Ohio, Illinois, Pennsylvania and New York. It does not measure transaction value, and it does not distinguish a portfolio sale from a single-facility sale. It is the most complete public record of who handed a licence to whom, which is a different and more verifiable thing than a deal announcement.
