Counts are changes of ownership recorded in CMS enrollment files for the covered care settings, as published; a single portfolio deal can appear as multiple facility-level records.

How transaction attribution works
CMS records ownership changes at the facility and enrollment level. WhoOwnsCare resolves the entities on each side to corporate families, preserving relationship types and confidence labels, so a set of facility-level records reads as what it usually is: one operator exiting a market and another entering.
